Preparing for Your Appointment

Step 1

Check your email for an invitation to our online patient portal through AdvancedMD. All new patient intake forms will be available through this portal. Please make sure to fill out all forms as completely as possible.

Step 2

Feel free to contact the office in advance to ask any questions you may have about the paperwork or your first appointment.

For office appointments, plan to arrive 15 minutes prior to your appointment to complete your initial check-in process.

For online appointments, log on to your private link 5 – 10 minutes early to sort out any mic or camera issues.
